BMHA Now Accepting Vaccine Receipts for Proof of Vaccination

On Thursday, September 9, ALLIANCE Hockey held a meeting with member associations (including BMHA) to discuss how its mandatory vaccination policy will be implemented, including proof of vaccination. The full policy can be found here.



The following participants are required to show proof of full vaccination by October 31, 2021, with their first dose received by September 17, 2021:

- Players born in 2009 or earlier (this includes players born in 2009 that have not yet turned 12)
Game officials (referees, linespersons, and timekeepers - submit proof of vaccination directly to ALLIANCE Hockey at https://alliancehockey.com/Forms/20020/)
Staff
Team officials (coaches, trainers, and managers)
Volunteers


Participants will be required to upload their vaccination receipts to the BMHA website where it will be stored securely. To upload your vaccination receipts, visit https://secure.brantfordminorhockey.com/Forms/5450/

Ontario Health Card holders can obtain their vaccine receipts at: https://covid19.ontariohealth.ca. This receipt does not display your full Health Card number and the only personal identifiers are your name and date of birth.

Participants can request a "medical exemption" or a "creed/religion exemption" through forms found here. These forms are the only accepted method to apply for an exemption. ALLIANCE Hockey has formed a committee that will review all exemption requests and provide a response within 72 hours. Those granted exemptions will be required to present a negative COVID-19 test result 24 hours prior to each ice time at their expense.
